Mission Statement To develop services using the person-centred planning model based on a clear care programme approach in line with the white paper: Valuing People. This means giving people with learning disabilities real choice and real opportunitiesthe same as those available to any other citizen. Introduction Threeways is a Residential Care Home Registered with the Care Quality Commission. The home provides accommodation and 24 hour care and support for 6 individuals with enduring Learning Disabilities and other complex needs.

Person Centered Planning Threeways values all its users as individuals, and believes that however severe their disability, they should be encouraged and enabled to live as normal and fulfilling a life as possible. The Person Centred Planning is central to the services that the home provides to users and encompasses the main areas of: Assessment of the Service Users needs; Planning through the Lifeplan meeting (a forum where the Service User, their Keyworker, parents, carers and professional staff gather to develop a plan to meet the Service Users strengths, needs and wants; followed by the Implementation of the plan and later its Evaluation and Review. Evaluation is on-going appraisal and reappraisal of the issues raised by the service user, their advocate and the multi disciplinary care team and occurs every six months or more frequently, according to the service users needs. All service users are offered opportunities for making choices in their daily activities. Service Users Profile of People living in threeways Threeways provides care for people with a learning disability who also have varying degrees of challenging behaviours, mental health problems, AUTISM and other complex psychological and emotional problems. Person What is the Dream / Night-mare Who is the person? What is the person good at? What are the persons needs? What is the plan of action? What is the story / History?

Leisure The home has its own transport that is suitably adapted to meet the needs of individual service users so that they may enjoy both domestic and leisure trips. Every opportunity is taken to ensure that service users go on regular holidays supervised by the staff as well as making use of local recreational facilities and entertainment spots. Membership of local churches further helps those who wish to become more closely involved in the local church community. Parents, relatives and advocates, as well as volunteers are welcome to visit the house on a regular basis. Individual friendship groups are encouraged. A range of leisure activities are available which the individual is encouraged to participate in. Day Facilities Daytime activities are tailored through the Service Users own individual Person Centred Planning to meet their needs and wishes. Service Users living at Threeways may travel to Shrewsbury Centre for day care and therapeutic activities. Social education, speech and language, alternative communications are typical of the therapies that are provided to meet the needs of this service user group. Service users may also enjoy many other supported learning provided at East Surrey College including art, music, horticulture, etc. East Surrey College is close by for other educational and vocational pursuits.

Support Services Support services for our service users are accessed from Kingsfield Centre where other professionals such as consultant psychiatrists, dietician, psychologist, and speech & language therapists are based. All service users living in our home have their own GP and are registered with a dentist. Other services such as chiropody and optician are accessed as and when necessary. Staff All care staff are trained and their performance and training needs are regularly reviewed. Training is provided both in-house and externally. Threeways is promoting the Qualification and Credit Frameworks to Level 2 and 3 Standards in Care for all of its support workers. Conduct and Management of Home Threeways is privately owned. Management of the home is the responsibility of the Manager, who is a well trained experienced professional. Both owners are qualified professionals.

Location Salfords is a popular and sought-after residential town located immediately to the south of Redhill and within only a few miles from the M25 motorway and the Surrey/Sussex green belt. There is a good range of shops, restaurants and leisure facilities within easy reach in Redhill with theatres, cinemas and shops only a couple of miles away. Crawley is also only a few minutes away. The Property A substantial brand 6-bedroomed detached property with the highest specifications. It is on the main road and occupies a central position. There are regular bus services and the Salfords mainline rail station is down the road. Gatwick airport is only a few miles away. We are strongly committed to provide a very high standard of support on a 24 hour basis to our service users. Our aim is to provide a happy, homely, relaxed and safe atmosphere at all times, which is of the highest standard, and which promotes respect, privacy, and dignity and encourages independence at all times.
