About CCBRT HISTORY Vision 1994: CCBRT established to provide Community Based Rehabilitation (CBR) 1996:CBR services extended to Moshi 2001: Disability Hospital (DH) opened in Dar es Salaam on land donated by the Government of Tanzania (GoT) 2006:DH surgical capacity expanded 2007: CCBRT and GoT formalized a public-private partnership to establish and operate Maternity and Newborn Hospital (MH) 2011: CCBRT broke ground on MH A Tanzania where people have access to quality disability services as well as safe maternal and newborn healthcare Today: CCBRT has a 471 staff, CCBRT has more than 150 beds for in-client services at the Hospital. CCBRT has an 11-member general assembly, six board members, and nine senior management team members. The Maternity and Newborn Hospital is preparing for the final stage of construction and equipping; opening is slated for late 2017.

Disability Hospital Today Ophthalmology Consultations & Surgeries Orthopaedic Treatments & Surgeries Physical Rehabilitation Services Optical Assessments & Prescriptions Community Based Rehabilitation Activities Private Clinic Services Today, CCBRT Disability Hospital is a 230 bed hospital in Dar es Salaam with a network of partner facilities around the country. Annually, conduct over 100,000 consultations & nearly 9,000 surgeries each year. CCBRT aims to serve vulnerable populations and offers subsidised care and treatment. Certain conditions, and all children under five years of age, are treated free of charge.

CCBRT Innovations in Healthcare: two challenges Challenge: Patients not accessing available, free care. Challenge: Inefficiencies in service deliver and limited culture of continuous improvement Factors: Cost of transportation to healthcare facility Lack of awareness of available treatment Cost of accommodation Factors: Routine quality improvement Scientific problem solving Standardized process and protocols Integrated monitoring and evaluation

Solution: TransportMyPatient Overview Impact Consistent increase in the number of patients accessing treatment On track for 1,000 in 2015; 339 in 2011 Correct referrals to CCBRT Patients seeking treatment from remote locations TransportMyPatient Video link: https://www.youtube.com/watch?v=EmIuvSKfmes Estimated: 3,000 women each year develop fistula Patients referred correctly to CCBRT (97%) actually have fistula 100% of patients eligible for transportation support receive it

Solution: Lean Management Overview Equip staff with skills to use scientific approach to problem solving Standardize existing treatment and process protocols, procedures, and trainings Coordinate monitoring and evaluation frameworks Pilot implementation of the Integrated Management System Impact Continuous quality improvement process implemented in eye department; patient flow revised
