Fig 1 Key surgical steps. (A) Before the tendon transfer, the posterior torn cuff is repaired (partially repaired). To reduce tension of the repair construct, the footprint of the cuff is medialized. (B) Using a narrow bone chisel, the pectoralis minor tendon is released with a flake of bone tip from the medial side of the coracoid. (C) Leading sutures are passed around the tendon-bone junction in the simple stitch configuration and tied. The leading sutures are retrieved from the space behind the coracoid process to the anterolateral portal. (D) The pectoralis minor tendon graft is fixed to the lesser tuberosity using a knotless anchor.
