(A) At level L5–S1, disc space narrowing with a low signal intensity of the disc is seen, with concomitant high signal intensity of the endplates of L4 and L5 (arrows). This is consistent with disc degeneration with Modic endplate changes. (B) At L5–S1, disc degeneration is present without endplate changes; the disc space narrowing is less pronounced than in (A). At level L3–4, however, the anterior corners of the endplates show focal high signal intensity despite a normal disc (arrows). This is based on osteitis secondary to axial spondyloarthritis.
