Fig 1 Pre- and postoperative diffusion-weighted magnetic resonance imaging (DW-MRI) of a 64-year-old male who was noted 4 years after Right carotid endarterectomy (CEA) to have recurrent Right carotid stenosis of 90%. Right carotid angioplasty and stenting (CAS) was performed and patient noted gait disturbance and diplopia postprocedure that resolved after 36 hours. DW-MRI done 24 hours after the procedure demonstrated 12 microemboli, six ipsilateral and six contralateral. Red arrows demonstrate punctate emboli in the Right temporal and Left occipital regions. Fig 2 Pre- and postoperative diffusion-weighted magnetic resonance imaging (DW-MRI) of a 62-year-old male with significant cardiac and pulmonary comorbidities with symptomatic 70% Left carotid stenosis. Patient underwent Left carotid angioplasty and stenting (CAS) and was noted postprocedure to have Right sided upper extremity weakness that resolved after 24 hours. Postop DW-MRI demonstrated 20 microemboli, 19 ipsilateral, and one contralateral to the index lesion. Red arrows demonstrate punctate emboli in the Left frontal and parietal regions and the Right parietal region.
