Minimum Necessary Standards Disclosures of and Requests for Protected Health Information Non-Routine Situations Disclosures in response to requests from selected persons Disclosures in response to all other requests Requests for protected health information from others

Minimum Necessary Standards Special procedures for using, disclosing or requesting the entire medical record. Special requirements for using, disclosing or requesting certain types of information.

Minimum Necessary Standards Exceptions Requesting PHI from or disclosing PHI to another health care provider for treatment purposes. Disclosing PHI to the client or to a personal representative who is authorized to make health care decisions for the client or the client’s estate.
