Infection Prevention & Control MRSA Bacteraemias- trust target - 0 There is a procedure for taking blood cultures that must be followed (ANTT), Contaminated specimens are a pain in general but with respect to MRSA lead to avoidable fines. Don’t forget decontamination treatment If known MRSA positive patient then ADJUST EMPIRIC REGIME to cover (it may avoid bacteraemia) CDI - 59 cases - 2015-2016 now breaching target CPE and MDRO There is a screening policy in position to ensure cases are detected Your role is to make sure that it is implemented Where CPE is a significant risk or is known the formulary will not cover call microbiology

Infection Prevention & Control Antibiotic prescribing- legible documentation Initial prescription of 5 days 24 and 48-hour review of antibiotic prescription Review potential to change IV to oral (CHORAL) Review microbiology results and de-escalate antibiotics When rewriting after 5 days put the stop date in Talk safe initiative/reflecting on unsafe practices- Hand Hygiene Covert audits and performance management in place It may come from nurses, pharmacists or AHPs Reflection on practice Bare or appropriate PPE covered below the elbows

Contacting Microbiology Consultants Your own team is the first source of advice In hours use NERVECENTRE it gets us wherever we are in the hospital OOH discuss with your clinical team who will contact microbiology service often simple problems such as alternatives to penicillin and oral regimes can be dealt with by them
