Federal and State Requirements Vaccine name, date of administration, name of manufacturer, lot number, expiration date, name and title of person who administered vaccine, name and address of where vaccine was given, date that vaccine information sheet was given, publication date of vaccination sheet that was given State: Patient identification number, Patient first and last name, Patient date of birth, Patient address, Patient race, Patient gender, Vaccine for Children program eligibility, if the patient is eligible for the Vaccine for Children program, Dose at the administration level under the Vaccine for Children program, if the patient is eligible for the Vaccine for Children program, Vaccination presentation or vaccination code using approved Immunization Information System (IIS) code type.
