5 Children’s Waivers CES (Children’s Extensive Support) CHCBS (Children’s Home & Community Based Services) CWA (Children with Autism) CLLI Children with Life Limiting Illness) CHRP (Children’s Habilitation Residential Program) Child must meet following criteria for all waivers: Financial Functional Targeting USA citizen

Financial Criteria Child cannot have more than $2000 in assets Child’s monthly income must be less than $2199 for 2015 (3 times the SSI rate - $721 for 2014, $733 for 2015).

Functional Criteria Long Term Care 100.2 Assessment: Bathing Dressing Toileting Eating Mobility Transferring Supervision Cognitive/Memory Supervision Behavioral Abilities are scored from 0-3. Child required to score a score of “2” in at least two ADL areas or a score of “2” in at least one of the Supervision areas.

Targeting Criteria Different for each Waiver program based on purpose for each Waiver

CES – Children’s Extensive Support Waiver Who served? Children birth – 17 years Children with intensive behavioral &/or medical needs who are at risk of institutionalization Children birth-age 4 must have a developmental delay, Children 5-17 must have a developmental disability Child must meet ICF-MR level of care Child must have actual medical condition or serious behavioral condition such as aggression toward self, others or property which requires a direct physical intervention every 2 hours during the day and every 3 hours during the night – YES, THERE IS NIGHT TIME CRITERIA Children may or may not be eligible for Medicaid via another program

Waiver Services con’t Assistive Technology Adaptive Recreation fees and services Vehicle modifications Community Connector Note: Services only available when natural supports, Medicaid state plan benefits or other funding are not available

So you heard there is no waiting list? Technically, there is no wait list However….. Children are enrolled in the order they went on the waitlist. Metro area CCBs are backlogged in enrollment. High volume of children applying Can be a fairly significant wait time before child can enroll

CHCBS – Children’s Home & Community Based Services Waiver Who served? Children birth-17 years Children who have intensive and high cost medical needs Children must meet a skilled nursing level of care Children who are not eligible for Medicaid through another program

CHCBS Waiver Services Case Management In Home Support Services (IHSS)

Is there a wait list There is no waitlist at this time for the CHCBS waiver

CWA – Children with Autism Waiver Who served? Children birth – 5 years Children who have an Autism diagnosis (299.00) Children may or may not be eligible for Medicaid via another program

CWA Waiver Services Behavioral Intervention

Is there a wait list? There is currently a waitlist. There is a proposed bill to eliminate the waitlist. If it passes, children will begin enrolling in July 2015 Children are prioritized on waitlist by their Adaptive Behavior Composite Score so may move up & down on the waitlist

CLLI – Children with Life Limiting Illness Who served? Children birth – 18 years Children who have a life limiting diagnosis (i.e based on opinion of medical specialist, that child has a prognosis of death before the child reaches adulthood) Child must meet a hospital level of care Child may or may not be eligible for Medicaid via another program

CLLI Waiver Services Respite Counseling Expressive Therapy Palliative/Support Care

Children birth – 21 years Children who are placed through a county Department of Social Services Children who have a developmental disability Children who have extraordinary service needs and for whom services cannot be provided at the county negotiate rate Child must be at risk of institutionalization Child must meet ICF-MR level of care
