Designing Service Delivery to Address Disparities Integrated care teams – Medical provider, behavioral health specialist, adherence nurse care manager, insurance navigator, referral coordinator, lawyer, community health worker/retention specialist, nutritionist, etc.
“Mind and Mouth” Services Mind – Behavioral Health Psychiatry Mental Health Therapy – Individual, Group, Peer Support Substance Use Disorder Treatment – Outpatient Acute Stabilization, Psychotherapy Group, Relapse Prevention, Medication Assisted Treatment (abstinence and harm reduction options) Screening and Assessment Peer Support Mouth Preventive Care (cleanings, etc) Restorative / Comprehensive Care (root canals, dentures, etc) Aesthetics (whitening, etc)
Service Delivery Integrating “Mind and Mouth” to Improve Health Mind – Behavioral Health Specialists embedded in medical team Crisis intervention / ER diversion Immediate connection for entry to behavioral health services Short term therapy to bridge transitions (into/out of care, life events) Screening and assessment Evaluation and support letters for gender affirming surgery Mouth – Significant expansion of capacity with the opening of 1525 Primary care reminders about making dental appointment Dental screenings to promote health – understanding whole person (oral cancers, HIV, etc) Cost/coverage impacts access DC Medicaid adult dental coverage vs Medicare / commercial and no coverage Sliding Fee Scale as FQHC – no cost or very low cost preventive visits; percentage fees for restorative work Ryan White funds (HIV+ patients) Wellness Services – Examples of specific enhancements to improve health outcomes / improve use of services Dental referrals to smoking cessation programs Acupuncture for managing pain instead of medication to access dental work or managing chronic pain instead of medication for patient in recovery program
