What to expect - Summary Hospital Care Recovery/Therapy if needed Estimated date of discharge Discussion with ward staff re ongoing needs Referral to Hospital Social Worker if required An assessment of need undertaken Discussion of how best we can support you on your return to usual residence Consideration of alternative placements if required Services arranged/signposted Discharged

Going home – What needs to be considered How will you manage? Can you manage? Who is around to support and assist you? What do you want? What do you need? No ongoing support required – Good circle of support Ongoing therapy/temporary support required Long term support required Short term placement required Support with housing/social needs District Nursing needs Aids and equipment Medication Transport

Assessment of need A trusted assessor will visit you on the ward to discuss your needs Your opinions, preferences and wishes will be taken into account Relatives, carers or advocate will be involved if you wish Medical opinions and observations will be taken into account The trusted assessor will agree a plan of care with you You will be provided with information/contact numbers Arrangements will be made A financial assessment will be undertaken (or at a future date)

Potential ongoing support Rehabilitation in one of our bed bases – time limited Rehabilitation at home (no previous package of care) Practical and personal care, therapy – time limited Intermediate Care at Home – Practical and personal care, can include support from Nurses, Physios, Occupational Therapists – time limited Respite Care – In a residential or nursing home – time limited Support from Red Cross/WHIA/Housing Association Assessment and provision of equipment that will support you Package of care – From a care provider Day Care Referral for a Link Worker
