Introduction This should be a brief introduction to the case, but should not include the final diagnosis.

Case Presentation Presents the data about the patient. Information in this section should include: Description of why the patient sought medical help. Observations of the medical exam. Description and results of any specialized tests that were performed.

Case Management Document the management and outcome of the case by explaining the treatment plan for the patient. Outline what was done and the outcome. Details should include: Length of treatment Frequency of treatment Type of treatment and method Measurement of progress or lack of progress How and why the treatment ended
