Uses of height & weight : height & weight measurements provide  Valuable data about your patient’s growth and development.  Nutritional status.  Overall general health.  Accurate assessment of height and weight is also a vital assessment used to determine appropriate medication dosages.

Con…  and to evaluate the effectiveness of drug, fluid, and nutritional therapy.

Examples of uses  Weights above the normal range may indicate obesity or fluid retention.  Weights below the normal range may indicate malnutrition, delayed growth and development.  cachexia (weight loss marked by weakness and emaciation that usually occurs with a chronic illness such as tuberculosis or cancer).  Height is compared with weight to evaluate growth of infants and children.

Expresses of height  Measurement of height is expressed in inches (in.), feet(ft), centimeters (cm), or meters (m).  1 in. = 2.5 cm 1 cm = 0.4 in.  1 ft = 30.5 cm or 0.3 m  1 m = 39.4 in. or 3.28 ft

Expresses of weight  Measurement of weight is expressed in ounces (oz),pounds (lb), grams (g), or kilograms (kg);  1 lb = 0.45 kg 1 kg = 2.2 lb  1 oz = 28.4 g 1 g = 0.35 oz

Nursing role  If serial weights are being done, weigh the patient on the same scale at the same time of day.  Be sure to document the measurements in centimeters or inches, kilograms or pounds.  Because children have frequent changes in growth and development, their measurements are often documented on growth charts for easy monitoring.

Categories of BMI for Adults Age 20 or Over Reference ranges for a desirable BMI are: men: 20.5–25.0 women: 19–24 25–30 = overweight above 30 = obese

Other Reference BMI Weight Status : Below 18.5 Underweight 18.5 – 24.9 Normal 25.0 – 29.9 Overweight 30.0 and Above Obese

Documentation  The height and weight measurements are recorded on the appropriate form, such as the admit assessment form.  Daily weights are usually recorded on the vital signs record.  If the weight is taken at a different time or on a different scale, the variation should be recorded.

Exercise  The BMI chart is not for babies, children, teens, pregnant or breast feeding women, adults over 65 years old, very muscular people and endurance athletes such as runners, WHY ?
