3 As children enter custody in the Northeast Region, the placement specialists will discuss children who pose potential risk behaviors in the area of Danger to Others (DTO), Sexual Aggression (SA), and/or Sexually Reactive (SR) behaviors with the CANS consultant.

4  The Well Being Unit will assist the Regional CANS Consultant in identifying children age 5 or older requiring a CANS be completed.  The Well Being Triage Team, consisting of the HAR, Educational Specialist/Consultant, Placement Team Leader, Fiscal Director, Nurse, Regional Psychologist, Interdependent Living Specialist, Social Work Practice Specialist, SAT Coordinator, CANS Consultant, and DRA.

5  During Triage (& MDT), the team will ask the FSW if a CANS has been completed and the current placement of the child. Additionally, we will determine if the child has an “actionable service score” (2 or 3) on Danger to Others (DTO); Sexually Aggressive (SA); and/or Sexually Reactive. If a CANS has not been completed, the FSW will be instructed to complete and submit the CANS within two business days to the CANS Consultant. The CANS consultant will notify the High Risk review team if a child has an “actionable service score” of 2 or 3 in DTO, SA, and/or SR.

6  When a child enters custody and upon receipt of an initial CANS for approval, the CANS Consultant will review any child that has an “actionable service score” (2 or 3) on Danger to Others (DTO); Sexually Aggressive (SA); and /or Sexually Reactive (SR).

7 At that time, the CANS Consultant will: 1. Notify the FSW and Team Leader of the child who has scored a 2 or 3 in DTO, SA, and/or SR and include the High Risk Spreadsheet instructing them to complete and return to her within 3 business days. The email will explain that the child falls into a category of the Brian A. settlement agreement in which DCS is committed to identifying and reviewing the placement of any child who might pose a high risk for violence or sexual assault on other children in any resource home (whether in custody or not). 2. Notify the DRA and the Placement Team Leader if a child has been placed in a foster home that has a score of 2 or 3 on DTO, SR, and/or SA.

8 Upon notification from the CANS Consultant, the Placement Team Leader will: 1. Determine if the child is presently placed in a resource home in which there are other children, and ascertain the number, sex, and ages of foster/birth children currently in the home. 2. Provide a copy of the High Risk Spreadsheet to the private provider agency for their completion and input. 3. Prior to a child with an “actionable service score” of 2 or 3 in DTO, SA, and/or SR going on respite, the private provider and/or DCS Foster Parent Support will notify the Placement Team Leader. The private provider and/or DCS FPS will also ensure that a safety plan is in place and that during respite situations a copy of the safety plan follows the child to the respite home.

9 The CANS Consultant will develop a list weekly of children requiring a High Risk Review due to having an “actionable service score” on DTO, SA, and/or SR and being placed in a foster home setting with other children. These children will be reviewed during the WB Triage call on Monday and/or Tuesday mornings. The review team will consist of the WB Triage Team, FSW, TL and other members as deemed appropriate, such as the placement specialist. This list will be in addition to the customary well being triage list of children.

10 For children that are already in custody, but requiring a transitional, reassessment, trial home visit, or discharge CANS, when an actionable service score (2 or 3) in DTO; SR; and/or SA is present, the CANS consultant will notify the DRA and Placement Team Leader so that we can place the child on the WB Triage List. The DRA will notify the TL and FSW that the child has been placed on the list for review for that month. Additionally, the CANS Consultant will send the worksheet to the FSW to complete prior to the WB review.

11 The DRA will utilize the High Risk Spreadsheet provided by Central Office to record the names of the children identified in the region as having “actionable service scores” and placed in a foster home setting with other children as they are reviewed by the region’s High Risk Review Team. For each child, the DRA and the High Risk Review Team will provide Central Office with answers to the following questions:

12 1. What are the behaviors that identified the child as having high risk needs? 2. Were the Child and Family Teams of each of the children in the home aware of the placement of the high risk child? 3. Is each Child and Family Team comfortable with the safety of the placement? 4. In the time since the placement of the high risk child, has the region reviewed the placement and reviewed the circumstances of each of the children in the home? 5. What happened during the original placement that resulted in the high risk child being placed with other children despite having high risk needs? 6. If there is presently a safety concern for any child in the home regardless of the CANS score of the high risk child, how is that safety concern being addressed? 7. Has the region taken any action in response to what was learned from reviewing the high risk placement (e.g., training, refinement of the placement process)?

13 The region’s High Risk Review Team will provide Central Office with sufficient factual details in the region’s responses to these questions to allow Central Office to be satisfied that the placement is in fact appropriate or, in the event that it is not, that actions are being taken by the region to address any safety concerns.

14 Beginning October 1, 2010, the DRA will utilize the worksheet provided by Central Office to document answers to these questions and any additional information the region deems relevant over the course of the month as children identified with an “actionable service score” and placed in a foster home setting with other children are reviewed.

15 The DRA will provide Central Office with a copy of the completed worksheet for the month by no later than the 5 th business day of the following month. & The Northeast Region has a monthly phone conference with CO and our Brian A. monitor to ensure compliance & safety is paramount!
