1 1.The order is sent to HEO/Wiz when Home Infusion Device is marked as present on the Nursing Admission History Form. 2.You will see a pop-up window when opening HEO/Wiz : Select to Review or Defer. NEW PROCESS Providers will receive a pop-up in HEO/Wiz when a patient with a home infusion device has been identified. Medication infusion pumps seen at VUMC include but are not limited to: insulin, epoprostenol, treprostinil, baclofen, pain, and chemotherapy devices. Providers are encouraged to order the recommended consult. Consult team: Assesses patient/designated caregiver’s competency to manage the pump. Obtains a signed Patient Agreement document available via a pop-up during order entry. It can also be found via the blue link in the screen (see back) or E- Docs. Identifies the device, medication, dose and/or rate, route, and further instructions or comments as needed. Identification of Patients with Home Infusion Devices

2 If you choose Review: 3.Select Pump Status: Continue Pump As Directed or STOP Pump 4.Options vary based on medication pump ordered. This example asks for endocrinology service. Selecting “No” triggers pop-up to order consult from appropriate team. If a negative response is selected to one of these three questions, Pump Status will default back to “STOP Pump.” The pump can only be ordered to continue if functional and appropriate.
