4.  Five Rivers Health Centers began on May 23, 2011 as a medical home for the low-income patients of Dayton, Ohio by providing both primary and specialty care services to patients through the academic rotations for the Wright State University Boonshoft School of Medicine (WSUBSOM) residency and fellowship programs.  Approved on April 10, 2012 to be a (FQHC) Look -Alike.  Three former hospital residency clinic departments - Family Health Center, Medical Surgical Health Center, and the Center for Women's Health  Combined Five Rivers Health Centers, currently serves over 15,000 patients with over 52,000 visits in the Greater Dayton community.  Approximately 60-70 new prenatal patients per month

6.  Tracey Waller named 2012 Ohio FASD Initiative Professional Advocate of the Year Award at the Ohio Fetal Alcohol Spectrum Disorders annual Not a Single Drop Day on September 7th in Columbus.  Spreads FASD awareness and our ASBI efforts through the local community and State of Ohio
