2 Date and page number - 2 NNSA SC Complex Electrical Safety SME assessed contractor and SC Electrical Safety Programs and processes (LOTO, Energized Work Permit, Electrical Work Planning) in light of NFPA 70E (Jul 05). –Found real deficiencies, developed CAP and used NFPA 70E’s structure and specifics for recommendations to update programs. Two completely separate electrical incidents in one day (Jan 06). –One incident by a prime NNSA contractor. –Other involved two subcontractors. –Assigned by SC Director to perform a Type B “like” investigation.

4 Date and page number - 4 Type B “Like” Investigation Major Finds Contracting Officers, Contract Specialists and CORs require training on importance and specifics of ES&H requirements and use of ES&H SMEs. –Develop interim ES&H training. –Develop formal annual local training by SMEs –Update CO, COR appointment letters to emphasis ES&H requirements. –*Working to get this concept in DOE wide contracting training.

5 Date and page number - 5 Type B “Like” Investigation Major Finds Ensure adequate oversight. –Developed Work Authorization Program/Permit (includes security reviews and on the Intranet). –Funded fulltime contractor safety person. –Weekly formal ES&H inspections by Feds. –Required PHA for site maintenance contractor Davis Bacon projects.

6 Date and page number - 6 Type B “Like” Investigation Major Finds Strengthen configuration management to insure drawings (data) are available prior to construction and upon completion. –Updated project processes to provide contractors with current drawings. –Updated project processes to require as-built drawings. Other Initiatives Created Energized Work Permit, Contracted Arc Flash Analysis Set up interim NFPA 70E training for electricians, HVAC mechanics and alarm mechanics. Working on NFPA 70E certification training. Created a path for Fed and Contractor injury reporting.

ID and Controls Flow down and implementation of landlord safe work procedures –Clarity of Contracting safety terms for Multi-employer Relationships (NFPA 70E)
