1 DISCOVERING DIABETES INTERPROFESSIONAL WELLNESS CLINIC Jade Gamber, Stephanie Twait, Bryn Sugrue, Kylie Muntz, & David LaBelle Ferris State University

2 Introduction  Expanding of the Interprofessional Wellness Clinic held at MCO in Big Rapids, through FSU.  Currently, the clinic is predominately for older adult patients with type II diabetes.  Our expansion includes making it available for type I and type II diabetics of all ages.

3 “Discovering Diabetes”  We chose this name in hopes that the community and our patients will truly learn what their disease is about.  MISSION: “It is our mission to improve the overall quality of life for diabetics of all ages living in the Big Rapids community. We strive to establish an intimate relationship amongst our cliental by providing compassionate, optimal, and specialized clinical care and education. This in turn will empower them to take control of their diabetes through the knowledge they acquire.

4 Vision  We plan to replicate this clinic by 2020 with Grand Valley students and faculty.

5 Philosophy  We believe that patient satisfaction and provider collaboration can be better achieved through a nurturing environment.  We believe in emphasizing the quality of our patient care and value the concept of patient empowerment through education.  With this mindset, we believe that our patient’s goals and an optimal lifestyle will be achieved.

6 Organizational Goals To educate, support, and improve the quality of life of diabetics of all ages that attend our clinic!

7 Organizational Objectives 1. To educate clients and their families specifically about what diabetes is, the complications that can arise from it, and the proper way to manage it upon their first visit. 2. To provide a comforting environment that assures a recurring, compassionate, and intimate relationship amongst clients, families, and our interdisciplinary staff that includes a 98% satisfaction rate measured by evaluations provided during visits. 3. To have 50% of our patient population’s A1C levels and cholesterol levels be within normal limits by proper education and disease management techniques.

8 Organizational Objectives con’t. 4.To provide an interdisciplinary clinic staffed with Ferris faculty and students of nursing, optometry, pharmacy, dental, and dietary and food service management as well as Grand Valley’s faculty and students in the Nurse Practitioner program. 5.To provide a clinic twice a week, four hours a day to accommodate patient’s schedules.

9 What Ferris faculty & students are already established?  Optometry Perform routine eye exams on patients Treat and evaluate patients ocular functions Provide education about vision r/t diabetes Potential complications from diabetes Introduce patient’s to other sectors at Discovering Diabetes  Nursing Provide knowledge about diabetes r/t  Disease process  Treatments  Glucose Monitoring Establish intimate relationships Demonstrate a positive image to a life with diabetes  Dental Hygiene Perform teeth cleanings X-rays Education about care of teeth/dentures for patients  Dietary Education al teaching about a well balanced diet  Protein  Carbohydrates  Fats Individualize diet plans Weight management & exercise Provide patient classes & online resources for patients.  Pharmacy Dispense prescription medications Give patient medication information Retain inventory & pharmacy files

11 Job Advertisement  To promote student volunteers from Grand Valley’s Nurse Practitioner program to gain essential knowledge and experience working with patients with diabetes.  For recruitment and community exposure  $18.75 for 75 flyers to handed out at the Cook DeVos Center for Health Sciences  $35 for an advertisement in the Grand Valley Lathorn
