Barriers to Care Transitions in the Elderly Experienced by Healthcare Providers: A Systematic Review Donna Volpe, RN-BC, MSN Doctorate of Nursing Practice Student, College of Nursing, The Pennsylvania State University Procedure Older adults are the largest consumers of healthcare in the U.S. (United States Census Bureau, 2011). This population will more than double between now and 2030 to over 70 million people (Centers for Disease Control and Prevention, 2011). Older adults typically suffer from multiple chronic conditions making them vulnerable to inefficient care and hospitalization. 19.6% of Medicare beneficiaries are re-hospitalized within 30 days at an estimated cost to Medicare of $17.4 billion dollars (Jencks, Williams, and Coleman, 2009). The purpose of this study is to synthesize current literature on barriers to transitions of care in the elderly. PICO Question Methods Design: Systematic Review Search Strategies: Databases: CINAHL, Medline (PubMed) and Google Scholar Key terms in search included transitions of care, elderly, discharge planning and Transitional Care Model Inclusion Criteria: English articles Articles focused on the elderly Exclusion Criteria: Duplicates Articles over 10 years Sample Selection Procedure Levels of Evidence 11of the 26 articles were a level of evidence rating of V or higher 15 of the articles were descriptive studies or expert opinion (Levels VI and VII) Studies were grouped based on thematic findings Summary of Studies Summary of Gaps in the Literature There is limited research on the perspective of the caregiver and provider More studies are needed with a focus on other potential members of the interdisciplinary team such as the undergraduate nurse, social worker, therapists, and nutritionist Most studies focus on the movement from hospital to home How do healthcare providers perceive barriers to care transitions experienced by adults aged 65 and older?
