2  Upon completion of this unit you should be able to: ◦ Describe the steps used in analyzing community assessment data ◦ Analyze actual community needs assessment data ◦ Identify the components of a community health diagnosis ◦ Write a community nursing diagnosis

3 Data analysis will identify current and potential stressors and determine the impact the stressors have on the health of the community During the assessment phase, data about the community core and the eight subsystems is collected The data comes from a several sources including: ◦ Direct observation- windshield survey ◦ Qualitative- interviews ◦ Quantitative – numerical or statistical data, facts

4  Data analysis can be completed in four steps ◦ Categorization- demographics, geographic characteristics, socioeconomic characteristics, and health services/resources. The Community-as- Partner Model serves as the framework for data analysis. ◦ Summarization- using narrative statements and numerical/statistical data ◦ Comparison- identify gaps, incongruencies, and missing data. Look for patterns or themes in the data. ◦ Inference elaboration- draw conclusions

5 ◦ Categorization  Put data into groups or categories such as demographics, geographic characteristics, socioeconomic characteristics, and health services/resources. The Community-as-Partner Model also serves as the framework for data analysis. ◦ Summarization  Summarize the data using narrative statements and numerical/statistical data. Put statistical data into charts or tables. Include observations discovered during the windshield survey

6 ◦ Comparison  Identify gaps, incongruencies, and missing data. Look for patterns or themes in the data. Work with a group to identify patterns that emerge in discussions about the data that was researched. Compare community level data to the parish, state and national level. This puts data into perspective. ◦ Inference elaboration  Draw logical conclusions about the data based on the patterns that have emerged. These conclusions will identify the strengths and weaknesses (stressors) of the community. From these conclusions you will develop a problem list. Once the list is made, cluster similar problems and prioritize them. This will lead to statements that will be the basis for the community nursing diagnosis.

7  A community nursing diagnosis is a narrative statement that: ◦ Describes and actual or potential problem ◦ Describes the population or group at risk ◦ Implies an etiology (cause or contributing factors) ◦ Gives evidence to support the problem statement (signs and symptoms)  The community nursing diagnosis should include the following narrative phrase: ◦ Risk of _______ among_________related to__________ as evidenced by____________________.

8  An example of a community nursing diagnosis is: ◦ Risk of infant mortality among all infants less than 1 year old in the community of Home Town, related to inadequate resources for prenatal care, and lack of obstetric and family practitioners within the community, as evidenced by a current infant mortality rate (IMR) of 24 per 1,000.  This diagnosis contains all four elements. The potential problem is identified (risk of infant mortality). The population is identified (infants less than 1 year). The cause is lack of resources. And the evidence to support the problem statement is the IMR.
