10 You can enter the Student ID number

11 OR You can enter the Last and First Name

12 Click Run Query

13 After typing in the Student ID Number or the Student Name you then must click on the drag down menu and select Discipline_Referral_Form

17 Enter your email name

18 Click on Lookup Admin Email

20 Click Submit for School Approval
