4 San Diego Affiliate What we believe: Everyone deserves the same opportunity to become breast cancer survivors and to have an assurance that their future healthcare needs are properly managed.

5 San Diego Affiliate Since 1982 progress has been made More early detection. Nearly 75 percent of women over 40 years old now receive regular mammograms. More hope. The five-year survival rate when caught before it spreads beyond the breast is now 98 percent, compared to 74 percent in 1982. More research. In 1982, the federal government committed $30 million to breast cancer research, treatment and prevention. Today, that figure is more than $900 million a year. More survivors. America’s 2.3 million breast cancer survivors, the largest group of cancer survivors in the U.S

6 San Diego Affiliate And yet….it’s not enough Globally, more than 11 million women will be diagnosed with breast cancer and more than 410,00 will die in 2008 In the United States: nearly 240,000 will be diagnosed More than 40,000 will die this year

7 San Diego Affiliate Keeping the promise: How do we work? Our promise: save lives and end breast cancer forever by empowering people, ensuring quality care for all and energizing science to discover the cures. To meet this promise, we rely on the information obtained through the Community Profile process to guide our work. The goal of the community profile is to gather information for the purpose of identifying gaps in services that must be reduced to improve breast health among members of the community and identifying the populations most in need of breast health services.

9 San Diego Affiliate Outcome-based Grant Making Susan G. Komen for the Cure® is focused on reducing breast cancer incidence and mortality within the next decade with increased emphasis on finding solutions for disparities in breast cancer across populations. Community grants will be made to support outcome- based strategies and promising practices to reduce disparities in breast cancer mortality in San Diego County.

10 San Diego Affiliate Areas of Focus Financial and Navigation Issues: Increase the availability of financial resources for diagnosis, treatment and post-treatment in San Diego County among uninsured and underinsured women. Partner or collaborate on resources to help patients and their families’ access and navigate the resources that are available.

11 San Diego Affiliate Providers: Increase competency among providers for breast health protocols and cultural/linguistic sensitivity. Partner with professional organizations to study/create/implement culturally sensitive continuing education and track progress.

12 San Diego Affiliate Underserved Populations: Programs that seek to reduce late stage diagnosis and mortality among underserved groups of women. Many groups of women are currently at higher risk due to lack of education about breast health, cultural/linguistic barriers, lack of available diagnostic services and late stage diagnosis. There are proportionally more such underserved women in the African American, Latina, Pacific Islander, Asian, Lesbian/Transgender and Homeless communities.

13 San Diego Affiliate Young Women at Risk: Provide screening, diagnostic, treatment and support services for women under 40 with a family history or palpable mass; to decrease late stage diagnosis and mortality rate by increasing partnerships with organizations that predominantly serve young women, especially under served young women; to increase awareness of breast health and breast cancer and the availability of culturally appropriate and relevant breast health materials for young women.

14 San Diego Affiliate Program Priorities 1) Increase the numbers of rarely screened or never screened women from the priority populations to breast cancer screening and diagnostic services 2) Improving the access of women diagnosed with breast cancer from priority populations to treatment and/or treatment support services to ensure successful treatment outcomes.

15 San Diego Affiliate Examples of Activities Guiding patients through the breast health system to facilitate and increase access to quality care Transportation and childcare Meal delivery for the patient and her/his family Linking to free and low-cost services Patient advocacy Interpretation services Helping women find providers near where they live Conducting provider training on cultural and linguistic aspects of priority populations Working with providers to ensure rarely or never screened women have access to services

16 San Diego Affiliate Examples of Activities Educational interventions aimed at increasing awareness of risk of getting breast cancer, the benefits of early detection, knowledge of available breast health services and eligibility criteria, and ability to access and utilize breast health services. Culturally and linguistically competent educational approaches that motivate and directly link rarely or never screened women to breast health services. Community-based outreach and education (i.e. alternative hours, services provided at locations where priority populations live, work, play and worship)

17 San Diego Affiliate Examples of Activities Guiding patients through the breast health/cancer system to facilitate and increase access to quality care. Assistance in obtaining support services. Meal delivery, transportation and childcare Patient advocacy Interpretation services Financial assistance Working with healthcare community and insurance companies to improve access to timely quality treatment and support services for women from the priority populations

18 San Diego Affiliate Areas of Future Study Investigation of the demographics of women in cities with the highest breast cancer incidence and mortality; Exploration of the relationship between income and breast cancer incidence and mortality and Exploring the needs of various minority groups in more detail.

20 San Diego Affiliate Guidelines and Instructions Qualifications: Applications will be accepted from U.S. nonprofit (federally tax-exempt) organizations, e.g., educational institutions, government agencies, and Indian tribes. Active members of the San Diego Affiliate Board of Directors must not be project managers or directly associated with a grant program funded by the San Diego Affiliate. Applications must be submitted in English. Applicant organizations must be located and/or provide services that benefit people residing in San Diego County. Funding Limits: Due to an increase in demand and limited funding, screening/diagnostic grants must be capped at $80,000 and Education/Outreach grants at $40,000.

21 San Diego Affiliate Restrictions: The project must be specific to breast health and/or breast cancer (e.g. if a project is a combined breast/cervical cancer project, funding may only be requested for the breast cancer portion). Services provided must benefit people residing in San Diego County. Salaries, if requested, are for personnel related to this project only and not general work of applicant. The grant program should be managed by any project personnel with the exception of family members whose compensation is directly or indirectly influenced by the project/program director.

22 San Diego Affiliate Funds may be used for : Salaries and fringe benefits for program staff Consultant fees and stipends Educational outreach costs and materials Supplies: educational, office, meetings Travel as necessary to the delivery of services or education Meeting and conference costs as are necessary to the delivery of services or education Indirect costs, if applicable, should be no more than 15% of direct costs. Equipment costs are limited to $5,000.

23 San Diego Affiliate Funds may not be used for: Medical research Scholarships or fellowships Construction or renovation of facilities Political campaigns or lobbying Endowments Debt reduction Food/Refreshments (As a non-profit, Komen San Diego seeks to always receive refreshments and entertainment from in-kind donors. We expect grantees to do the same)

24 San Diego Affiliate Returning Grantees: If you are a returning Grantee, please describe the efforts you have undertaken to become self-sustaining and/or to have this project included in your organization’s larger ongoing budget. (For instance, if you are part of a large medical system, and you are a recipient of successive grants, have you included the program in your organization’s budget planning cycle? If not, why not? If yes, please detail the outcomes.)

25 San Diego Affiliate Educational Materials : Funding to purchase educational materials should be included in your project budget. Grantees are eligible to receive a discount on materials and will be enrolled in a discount program through Susan G. Komen for the Cure®. It is expected that you use only Susan G. Komen for the Cure® educational materials as part of the administration of the grant.

27 San Diego Affiliate The Grant Application Proposal Cover Pages The proposal cover pages consist of general information about the organization and provide space for an abstract of the proposed project. If the organization is funded, this description will be used to describe the project in all publicity and reports. Abstract not to exceed 200 words. It’s the “advertisement” for why this grant program should be funded.

The expected result is stated in numbers or percentages. Measurable: The objective can be measured and how it will be measured is described. Achievable: Are the objectives you set achievable and attainable? Realistic: Can the objectives realistically be achieved with the resources you have? Time: The objectives can be accomplished

31 San Diego Affiliate Organization Capability Breast health/breast cancer services currently provided by the organization Experience in providing the proposed services and any related services History of serving the population Evidence of success in delivering services – and changing behaviors – in the proposed population. Ability to conduct program evaluation and quality management. Ability to coordinate activities with other providers of related services. Fiscal capability to manage proposed services and to ensure an adequate audit trail. Board of Directors: A complete list of your Board of Directors (and your fiscal sponsors, if applicable)

32 San Diego Affiliate Problem Statement & Needs A demographic description of the population served: ages/ethnicity/income if known/residence zip codes. Evidence of risk/need within that population. Statistics specific to San Diego County (if local data is unavailable, include other applicable data)

33 San Diego Affiliate Project Description/Work Plan Explanation of the proposed project and provide thoughtful details on how it addresses one or more of the gaps identified in the Statement of Need. It is not enough to list the population – you must “connect the dots” between the need as stated, and the desired outcome. Description of project goals and objectives. (NOTE: When writing your objectives, please differentiate between the number of new clients who will be served and the number of clients already enrolled in your program who will be served again during this grant period.

34 San Diego Affiliate Description of Activities Describe how your approach uses outcome-based strategies or promising practices. Explain why you chose this approach (is this a new or ongoing activities of your organization?) Outcome-based strategies are programs that have been implemented, evaluated, and found to be effective in improving access, promoting behavioral change and/or empowering individuals to make good breast health decisions. Promising Practices are defined as innovative approaches that are likely to be effective but have not yet been fully evaluated. They may also be called “emerging best practices.”

35 San Diego Affiliate Description of other organizations or entities, if any, participating in the project. If applicable, letters of collaboration should be included from each organization). If collaboration with another organization or entity is “substantive” a signed Memorandum of Understanding (MOU) between both parties should be included. A review of comparable programs offered in this service area and an explanation of how this project is unique. Description of a clearly-defined evaluation plan (This section should describe how you will monitor the progress you expect to make in the project and how you will assess and document the impact of your program).

If you do not have audited statements, skip this step. Copy of your organization’s current annual budget. List of other sources of funding for the project, amounts received or to be received from these sources.
