3 Five year partnership Improve health and wellbeing of Rotherham community –Reduce risk of heart disease and heart health inequalities –Improve awareness of and access to services available – from NHS, local authority, BHF and other voluntary/community sector organisations –Support for family members and carers of people with heart disease Not just an initiative for the public and voluntary sector – business involvement is vital Rotherham is an exemplar for heart health

4 Health and Wellbeing Strategy Joint Health and Wellbeing Strategy for Rotherham, in response to the Health and Social Care Act 2012 Health and Wellbeing Board co-produced with key stakeholders inc; RMBC, Clinical Commissioning Group, Public Health and Voluntary Sector Approved for public consultation by the Health and Wellbeing Board June 2012
