9 © 2015 Kauffman Scholars, Inc. www.kauffmanscholars.org Incentives All eligible students will need to complete and submit both the Incentives Application and a federal W-9 form in order for incentive requests to be processed: –Graduation/Completion Incentive; –Outside Scholarships over $1000; The deadline to provide the completed forms will be aligned with the grade submission deadline of June 15 th (tentative date); Target date for each round of incentives to be delivered is around August 1 st.

10 © 2015 Kauffman Scholars, Inc. www.kauffmanscholars.org Incentives (2) Students must be in GOOD STANDING at the time of application OR return to GOOD STANDING after the annual evaluation period; Incomplete applications will not be processed, NO EXCEPTIONS; Incentive awards will be mailed to the address provided on the updated W-9.

12 © 2015 Kauffman Scholars, Inc. www.kauffmanscholars.org PS Program Components Program Status –Active student (e.g. Good Standing, Watch, Probation); –Inactive student (e.g. Dismissal, Withdrawal); Scholarship Eligibility –Submission of all requested paperwork to KSI and the institution (when applicable) by stated deadlines; –A student can be program active, but not scholarship eligible; Postsecondary Incentives –Graduation/Completion; –Outside scholarships; –Only students in Good Standing qualify for incentives.

Scholarship Eligibility Why is it important? –Determines what students are qualified to be scholarship eligible each academic semester; –KSI deadline from schools to provide that list; Reasons for ineligibility; –Non submission of documents; –Non fulfilling verification obligations; –Completing the FAFSA after March 1 st ; –Loss of Federal Financial Aid; Can scholarship coverage be restored? –Yes, reevaluated prior to the start of the subsequent semester; –Student must remain engaged throughout semester of non- coverage.

18 © 2015 Kauffman Scholars, Inc. www.kauffmanscholars.org Scholarship Eligibility (2) In order to be scholarship eligible for the fall 2015 semester, ALL documentation below was to be submitted to by July 10 th : –Submitted 2015-16 FAFSA Confirmation; –Submitted 2015-16 Student Aid Report; –Financial Aid Verification Documentation (if applicable); –Fall 2015 Award Letter; –Fall 2015 Course Schedule; –Updated FERPA release on file (vary by school); Any student that failed to provide this information by the deadline is not be scholarship this semester.

21 © 2015 Kauffman Scholars, Inc. Status Process Good Standing - A student is considered to be in Good Standing with KSI when he/she is meeting all KSI behavioral, academic, and program expectations. Watch - A student will be placed on Watch if and when any of the Level 1 Actions occur. A student shall remain on WATCH for one (1) full academic year. Probation - A student will be placed on Probation if and when any of the Level 2 Actions occur, or when he/she fails to return to Good Standing at the conclusion of an academic year on Watch. Dismissal - A student will be subject to DISMISSAL if and when any of the Level 3 Actions occur, or when a student fails to return to Good Standing after one academic year on Probation.
