2. SPS Children’s Collaborative Goals By June 2010… Reduce overall Adverse Drug Events across all 8 hospitals by 33%; and Reduce surgical site infections in designated cardiac, neurosurgery and orthopedic procedures by 50%.

3. What have we accomplished? (ADEs) All hospitals have adopted the ADE trigger tool methodology Positive trend towards ADE reduction and meeting our goal of 33% reduction by June 30, 2010 Shared data definitions and methodology –Dissemination of an objective, reproducible measure of medication errors not dependent on self reporting Agreement to be transparent Shared goals and aims Collaborative relationships established Building of Inter Rater Reliability Agree on key driver for opiate over sedation reduction –Near agreement re state-wide standardized order set guideline
