1 ACT On Alzheimer's: Every Minnesotan Makes A Difference

2 Act on Alzheimer's ACT on Alzheimer's is a statewide, voluntary collaboration that includes over 50 organizations and 150 individuals seeking to prepare for the budgetary, social, and personal impacts of Alzheimer’s disease.

3 Genesis of ACT on Alzheimer’s 2009 Legislative Mandate for Alzheimer’s Disease Working Group (ADWG). Legislative Report Filed in January 2011. ACT on Alzheimer’s is second generation of work focused on implementation.

4 COLLABORATIVE STRUCTURE No one organization owns/operates the initiative: Leadership Council (36: Advisors, Strategists, Champions) Management Steering Committee (10: Organizations Actively Involved in Day to Day Operations; Make Decisions on Cross Cutting Issues) Operations (150+: Executive, Project Manager, Five Leadership Groups, Communications, Fiscal Sponsor, Host)

9 ENGAGE AND EQUIP COMMUNITIES FOR THE IMPACT ACT on Alzheimer's has developed a Community Tool Kit to assist communities in becoming dementia capable so they can support their residents who are touched by Alzheimer’s disease.

10 DEMENTIA CAPABLE COMMUNITY A dementia capable community is informed, safe and respectful of individuals with dementia and their families, provides supportive options, and fosters quality of life.

11 DEMENTIA CAPABLE COMMUNITY Community Awareness Diagnosis, Medical Management and Quality Care Information, Counseling, Education, Life Planning Community Supports Local Government Planning

12 DEVELOPING DEMENTIA CAPABLE COMMUNITIES A tool kit is available and can be used for: Convening key community leaders and members to form a Community Team. Assessing current strengths and gaps in dementia capability. Identifying community goals and planning possible ways to act. Acting Together to pursue priority goals and foster community readiness for dementia.
