3 Myth busting There is no such thing as a care record All records of care aren’t electronic Electronic records of care are not routinely shared between clinicians You have the right to see your care record/s* Repetition, repetition, repetition!!

4 Where we are now? Suffolk Shared Care Record has started! Paper mountains are being digitised More than 5000 shared records were viewed in urgent care in Suffolk last month 97% of patients can request digital access to their GP record today Repetition still exists

5 Fast forward a year…. Suffolk Shared Care Record will be used routinely across health and social care Every clinician will be able to access relevant information for the patient in front of them Repetition reduced

6 So what? You will be able to access your full care record ….which means you are better informed ….which means you can look after yourself better You can choose to allow your carer to access your record You / carer can communicate with your clinician through the record

7 What can you help with? What would you want to see on your care record? How do you want to interact with it? How do you want it organised? What is the useful stuff for you?
