2 Objective: To determine patient satisfaction with LASIK outcomes Methods: Retrospective analysis (Three Month post-operative email survey) 349 Patients had IntraLASIK surgery between January 1, 2008 and December 31, 2008. The Allegretto Wave 400 Hz Eye-Q Laser was the excimer platform used on all patients Survey Questions: 1.How would you rate the quality of your vision? (1 – 5 Scale) 1 = Poor 2 = Below Average 3 = Satisfactory 4 = Good 5 = Excellent Daytime Nighttime 2.Rate your level of Dry Eye Syndrome after LASIK 1 = Extreme 2 = Moderate 3 = Mild 4 = Same as before Sx 5 = Better than before Sx 3.Do you have chronic eye pain after your LASIK surgery? Retrospective Survey

3 Survey Questions Con’t. 4.Do you have difficulty with halos / glare? 5.Would you recommend this procedure to your family and friends? 6.Do you feel that the benefit of this surgery was worth the investment? 7.Rate the quality of life after LASIK 1 = Worse2 = Same 3 = Better 8.Rate the quality of your vision after LASIK compared to your vision with glasses and contacts 1 = Worse2 = Same 3 = Better 9.Would you have the surgery again knowing what you know now?

12 Conclusions A small percentage of patients are not pleased with the result of their LASIK procedure and may have problems with night vision, quality of overall vision, and/or chronic pain.
