1 COMMUNITY HEALTH IMPROVEMENT PLAN (CHIP) ANNUAL UPDATE REPORT Health Care Access Committee September 28, 2015

2 EVALUATE CHIP IMPLEMENTATION AND UPDATE CHIP WORKPLAN FOR FY 2015/2016 The annual update process includes a review of progress in achieving objectives and provides an opportunity to revise strategies being utilized. Tables containing the four priority areas with two identified challenges for each were prepared. As this is the third and final year for the CHIP, priority areas and objectives were not changed.

7 PUBLIC REVIEW AND COMMENT The Annual CHIP Update Report is currently posted to the DOH- Broward website for a ten day public review and comment period. Monthly, Quarterly, and Annual Progress reports are prepared and reviewed at several community meetings:  The Nutrition and Fitness Task Force  The Breastfeeding Coalition  TOUCH Partnership  Health Care Access Committee Quarterly CHIP Update Reports are posted to the DOH-Broward website.

8 ACCOMPLISHMENTS GoalObjectiveAccomplishment Priority #3 – Improve Maternal, Infant and Child Health Objective 3.3 Increase vaccination coverage levels for 7 th grade children by 1% annually. Increased community immunization outreach activities, continuation of the annual back to school community immunization points of dispensing, and collaboration with community partners have assisted Broward County to exceed the 95% target goal for the past two years. How it’s important for our community: Maintaining high rates of immunization for vaccine preventable diseases benefits the overall health of the community. Partnerships with schools, communities, and providers assisted Broward County to exceed the target goal of 95% of 7 th graders for the past two years.

9 ACCOMPLISHMENTS GoalObjectiveAccomplishment Priority #4 –Enhance Preventive Care Activities Objective 4.2 Reduce the proportion of Black adults who are obese by 5% annually. Implementing Healthiest Weight Florida activities have assisted in reducing obesity rates in Broward County by 9.8%. Black obesity rates were reduced by 35.4%, which is more than the 5% annual target goal. White obesity rates also decreased by 3.3% during the same period. How it’s important for our community? Undiagnosed and untreated health conditions can have serious health consequences. Prevention, diagnosis, reductions in obesity rates, and increased access to better nutrition including fresh fruits and vegetables can greatly improve quality of life and decrease premature death.

10 DEVELOP NEW PLANS FOR 2016 – 2019 A new Community Health Assessment (CHA) is being developed with a target completion date of May 31, 2016. The CHIP will be developed in June, 2016, based on the findings of the Community Health Assessment.
