8 Accomplishments Investments in capital projects – $70m to support 14 health centers since 2005 – $21m to support enhanced emergency department services Health Professional Loan Repayment Program – Providing support currently to 19 sites 19 primary care, 6 oral health, 5 behavioral health providers

9 Accomplishments Improved collaboration between BoMed and BoD and PCO to inform MUAs and HPSAs – 3 RD cycle of data collection from physicians regarding practice information during license renewal Primary care providers achieving Patient Centered Medical Home designation

10 Accomplishments Public and private investment in electronic health records and health information technology infrastructure Increased insurance coverage through state health benefit exchange, DC Health Link, and Medicaid expansion

11 Accomplishments Awarded State Innovation Model Design Award DC Million Hearts Learning Collaborative Increased integration of primary care with behavioral and dental heath services

12 Future Opportunities Operationalize and align PCMH efforts – Align QI projects with critical community health challenges – Standardize quality metrics – Promote PCMH as the place for patients to receive all health services and end fragmented care DOH supports innovation, DHCF sustains models that improve health outcomes, health care quality, lower cost, and improve patient experience of care
