3 The ACT O 11 th Grade Only O Districts MUST administer O Students who may choose not to participate: O Those for whom there are not sufficient accommodations available O Those who refuse or whose parents refuse for them to participate. O ADE has a form ready

4 The ACT O Children or relatives of the ACT testing coordinator are not eligible to test O From ACT: *Relatives and wards include children, stepchildren, grandchildren, nieces, nephews, siblings, in-laws, spouses, and persons under your guardianship O You may not supervise the administration of the ACT or have access to secure test materials, if any relative or ward will be testing on the same test date at any test center in the U.S., Puerto Rico, or Canada.
