The Health Roundtable KEY PROBLEM No clear governance structure or plan Leadership – difficult to maintain engagement with many competing agendas Difficult to get staff involved in pressure injury audits Limited local ownership of data Incident reporting was not consistent – staff did not feel their incidents reports were influencing change in practice Practice was often based on habits not best evidence 2

The Health Roundtable AIM OF THIS INNOVATION Improve patient outcomes & reduce harm Reduce incidence of HA pressure injuries and prevalence Consistent clear leadership Governance for reporting/ reviewing results Ensure pressure injury prevention is everyone's business Encourage interdisciplinary staff involvement in pressure injury audits and the value in doing them Local ownership of data Create and maintain a CULTURE of incident reporting Practice based best evidence not habits 3

The Health Roundtable KEY CHANGES IMPLEMENTED Timely Waterlow assessment Standardisation of continence & skin care management Completion of MST& appropriate referral to Dietician Protected Meal Times - promotion and implementation Practice Partnership model – eg regular rounding Appropriate equipment acquisition Creating a culture of Incident reporting via AIMS and pressure injury prevention Documentation ( pressure injury sticker, wound forms) Improvement in ICD -10 coding & documentation Pressure injury prevention is now everyone’s business! 4

The Health Roundtable WHAT CHANGES HAVE CONTRIBUTED TO THE SUCCESS? Engagement from all staff – from CEO KPI, Executive, Nursing Directors, interdisciplinary team and ward staff. Identification of pressure injury as a priority in Nursing Strategy 2010 – 2013 Tissue Viability Committee and interdisciplinary collaboration Standardising skin care and continence management & education PIP Champions on wards created local buy in – meet regularly Raised awareness Presentations to Grand Rounds & Senior Nurses Planning Day Creating and maintaining a culture of pressure injury prevention & incident reporting Involve the patient in their care and management plan Progressed from traditional habits to evidence based practice Publications, promotion and awareness 6
