The patient is disoriented and has a disturbed sleep-wake cycle. The nurse administers the Confusion Assessment Method (CAM) tool to differentiate among various cognitive disorders, primarily because 1.delirium can be reversed by treating the underlying causes. 2.depression is a common cause of dementia in older adults. 3.nursing care should be based on the cause of the cognitive impairment. 4.drug therapy with antipsychotic agents is indicated in the treatment of dementia.

An appropriate nursing intervention for the patient is to 1.let the patient know what behavior is socially appropriate. 2.assist the patient with all self-care to maintain self-esteem. 3.maintain familiar routines of sleep, meals, drug administration, and activities. 4.at every encounter with the patient, ask the day, time, and place to promote orientation.
