3 Must Pass Elements All six Must Pass Elements are required for recognition. Includes: 1A: Access During Office Hours 2D: Use Data for Population Management 3C: Manage Care 4A: Self-Care Process 5B: Referral Tracking and Follow-Up 6C: Implement Continuous Quality Improvement Total Must Pass Points = 29 (at 100% completion) © Qualidigm

4 Critical Factors These must be met for practices to receive more than minimal or, for some factors, any points. Includes: 1A-1*:Provide same-day appointments 1B-3: Provide timely clinical advice by telephone after hours 1G-2: Have regular team meetings / communication processes 3A-3: Report on a condition related to unhealthy behaviors / mental health / substance abuse 3D-1: Review and reconcile medications with patients / families 3E-2: Generate at least 75% of eligible prescriptions electronically 4A-3*:Develop self-management plans and goals 5A-1: Track lab tests 5A-2: Track imaging tests *Must-Pass Element © Qualidigm

5 Documentation Documented Process: the practice’s written procedures, protocols, processes, workflow forms (not explanations) Reports: aggregate data showing evidence of action Materials: prepared materials for patients or clinicians, e.g. clinical guidelines, self-management, and educational resources Screen shots: electronic “copy” used as examples (EHR capability) or materials (web site resources) © Qualidigm

14 Document Library Overview Overview: 1) Click “Load Document to Library” 2) Click “Browse” to select a document and enter the “Document Name” to identify it with the PCMH element 3) Click “Save and Close” 4) Once the document is uploaded, select “Add Link” to the right of the file in the Document Library 5) Indicate the number of reference pages, the relevance level (“Primary”), and select the appropriate PCMH element to link 6) Click “Save and Close” © Qualidigm
