Utilize popHealth to aggregate statewide clinical quality data – Develop infrastructure to accept QRDAs – Onboarding/testing of EHRs for submission of QRDAs Provide Non-EHR Incentive Payment providers along with those enrolled through the SLR a reimbursement for demonstrating quality of care – Modify SLR and PopHealth to be able to exchange CQMs – Modify SLR to accept CQMs for Medicaid providers who may not be applying for EHR incentive payments Allow provider-level analysis of Clinical Quality Measures – Provide a tool for user managed provider hierarchies: Organize providers into teams or groups Organize multiple teams Reporting at every level of the hierarchy – Provide quality performance monitoring: Allow comparison of multiple providers Allow comparison across groups of providers Provide the ability to identify ‘best’ performers and ‘under’ performers Analytics for States’ Use 9 Roadmap Goals - Wyoming

Short Term Objectives: 1. More control of MU Stage II Attestation Measure Submissions. 2. Refine and Improve the EP attestation workflow. (Ease of Use) 3. Staff Orientation into Clinical Analytics Long Term Opportunities and Expectations: 1. Business Services offering to Stakeholders/Healthcare partners. 2. Enterprise Level Analytics. 3. Targeted CQM point-to-point EHR Data Collection. 4. Fast Interoperable Healthcare Resource ("FIHR") and Public Application program Interfaces (API) Possibilities. 5 Open Source Application Alternatives. Analytics for States’ Use 10 Roadmap Goals - Alabama

Make PopHealth more useful for provider practices. – Get EHR data into PopHealth – Provide functionality to practices that leverages that data. – Get the data out of PopHealth, with push to our Texas HIEs the priority. Providers will configure the software to periodically reach out to their EHR and extract the information necessary to build a CCDA summary of care document for every Medicaid client. Alternately, CCDA docs would be procured via vendor-supported means, when available. PopHealth can then be configured by the provider to periodically push these CCDA docs to the HIE of their choice. HIEs will be able to offer profiles, downloadable from their website or pre-loaded in a PopHealth download. Analytics for States’ Use 11 Roadmap Goals - Texas

Goal 1 – Subgoals – Specific functionality Goal 2 Goal N Analytics for States’ Use 12 Roadmap Goals – Your State

Alabama indicated they have both short term and longer term goals – Are there specific times that goals must be met? – What determines when specific functionality is required? ONC regulations Funding opportunities Project schedules This temporal information is essential to determine prioritization & Milestones Identifying Milestones 13

Wyoming has stated the goal: – Utilize popHealth to aggregate statewide clinical quality data » Develop infrastructure to accept QRDAs » Onboarding/testing of EHRs for submission of QRDAs How does this differ from Connecticut’s primary goal? – Can we reach a common goal that we could then map to a set of action items to be completed? Coming to Consensus Goals 14
