2 Case Study Root Cause Analysis
Fishbone root cause analysis is used when: At team wants to identify what a student needs to learn in order to successfully transition. A team needs to study a problem/issue to determine the root cause. A team needs to identify areas for data collection. A team wants to study why a process is not performing properly or producing the designed results.

3 Draw the fishbone diagram.
List the problem on the head. Label each bone with categories to be studied. Identify the factors within each category that may be affecting the problem. Continue until you no longer get useful information. Analyze the results

What does the student need to learn in order to successfully achieve his post secondary goals? What questions do we still have about this student’s strengths and barriers? How will we gather information to answer our questions? What are the next steps?
