Inpatient Educational Services  One on one education to patient and caregiver  Assist providers with glycemic control  Establish a new plan of care for discharge  RN screens patient with elevated HgbA1C on insulin and escalates as necessary to the APN for management  Follow by phone once discharged until they see a primary provider

5. Survival Skills  Patient identified upon inpatient visit  Appointment made  Determination of Diabetic Self Management Education upon Survival Skills appointment
