3 * My idea for becoming paperless is to laminate all histories, MCHAT and ASQ’s. We could have maybe 10 copies around and give them to the patient’s family when they come in the door. The nurse or MA could collect them after working the patient up, enter the information, and then give it to the girls up front. They could clean it off and start the process over again. Thanks for considering my idea. * Jennifer Goodwin * JG, Chris and Machelle will be working together to find ways to implement this after DD returns.

4 * Billing and Insurance Department- Lottery Tickets * DeeDee, Susan and Pam * The billing and insurance department has been working hard to make sure that the self-scheduling flu clinic patients from the portal have insurance coverage for the vaccine. * They help us complete referrals in a timely manner by getting prior authorizations. * Pam helped us make sure the new handouts would be good to use for the assessments required for Medicaid patients.

5 * Jennifer Bremer- Wilco Card * Jennifer has done an amazing job of helping while Debbie D. is out. She has been working extra hours and also still makes time to take care of vaccine coordination. * NEXT MONTH: We will be offering a *NEW* Shout Out Gift Card. Bojangles!

6 * Machelle will be on vacation and unreachable. * Please let Dr. Henderson know if you have any questions or concerns related to IT issues. * If Dr. Henderson is not here, you may call her on her cell phone. * Jason is aware that I will not be here, and if necessary Dr. Henderson can call him. * Irene will be helping with portal questions, Sonya can go a “go to” person for clinical staff.

7 * Patients can schedule themselves from home. * Dr. Carter is the “flu” column. * There is a video online for parents (or you) that explains the process. * If a patient needs a physical form completed, please be sure the “Sports Physical Survey” is completed. We need to document that we’ve asked the screening questions.

10 * MU & PCMH Requirement: Electronic Access to Lab Results * The Patient Portal: Lab results will be available starting Monday. * Any labs ordered in house, once reviewed, will be marked as “completed” so they appear on the patient portal. * Please make sure lab results are appropriately ordered with correct results entered. * If there is a need for a “private” lab, mark as “clinical staff only”, or ask Machelle for help. * “With transparency, we need to double check accuracy.”

11 * MU & PCMH Requirement: Electronic Access to Lab Results * ASQ/MCHATS- if the patient is borderline, please write “concern identified” in the in lab results notes. “Concern identified for communication. WNL all others.” * If a patient is unable to cooperate with a hearing/vision screen: “Unable to cooperate.” * “With transparency, we need to double check accuracy.”

12 * MU & PCMH Requirement: Visit Summaries * The Patient Portal: Visit History will also be available on the patient portal. * Patient Exit notes will be sent to portal once note is finalized. * Providers may choose to enter patient specific information as they see fit. * Providers can also attach supplementary patient education handouts. * Providers are staying after the meeting for a quick review of this.

13 * Bright Futures is the standard for adolescent questionnaires. * For PCMH, we will need to provide documentation that we screen on a variety of items (mental health, nutrition, substance abuse, anemia, etc.) * Bright Futures Forms will be broken up into three age groups. * 11-14yr * 15-17yr * 18-21yr We will begin using the new forms September 17 th. These will replace the ones given in the office and made available online the same way ASQ’s are- yup, “Complete” the task!

14 * We all know about our “Menu Items” we will be reporting for Meaningful use such as demographics, structured clinical data, patient education, requesting electronic medical records, etc. * BUT- we also have to report Clinical Quality Measures

15 * MU isn’t just for pediatrics- its also used in adult populations. * There are many CQM’s- but because of our pediatric demographics, we can only report a few. * Tobacco Assessments (We do this already- Risk Assessments) * Tobacco Cessation * Adult Weight Screening and follow up. * Immunizations (We WIN!) * Asthma Assessment * Asthma Pharmacological Therapy * Controlling High Blood Pressure

16 * Right now: we are in Year Two of Stage One in Meaningful Use. * After we attest for Year Two- we will begin making more preparations for Stage Two. * Stage Two requires higher percentages for menu items. * We may be looking into a lab interface with Labcorp. * Messaging will be made available on the portal for non-emergent issues. * Providing electronic copies of health records.

17 * What is PCMH? * A team based health care delivery model that provides comprehensive and continuous medical care to patients with the goal of obtaining maximized health outcomes.

18 * Good News: PCMH and MU have similarities. * By meeting MU Stage 1, we will meet more of PCMH. * MU Stage 2 aligns with PCMH * For PCMH- we report the standardized care we develop the three clinical diseases. * We will be using the same CQM’s we use for MU for our PCHM diseases.
