3 Live Well at Home is a coordinated and integrated framework of practice helping older adults live at home longer.

5 5 Live Well at Home - Purpose Preserve client choice and control Extend community living for older adults Use private dollars for community-based services – early on Minimize “crisis” events Slow use of Medical Assistance

6 What is Live Well at Home? Support for Older Adults Who  Stay proactive about sustaining their community living goal  Learn to deal with personal risk factors known to cause a permanent move from home  Make plans and buy help with their own money  Stay connected to healthy aging programs, volunteers, providers and other important resources  Continue to make personal choices  Keep informal supports intact 6

7 How Live Well at Home Works The LWAH provider: Identifies risk factors Educates about risk factors and approaches Discusses their community living goal and community- based services & support Keeps family involved Conducts regularly scheduled follow-up on progress and successes with client 7

8 It is important to know… LWAH primarily focuses on the person’s community living goal. Ideas, action steps, and decisions are centered around the goal. 8

9 Risk Factors Causing Older Adults to Move From Home 1. Unable to perform everyday tasks 2. Had an injurious fall 3. No one available to help 4. Available help is stressed 5. Thinking of moving where there is help 6. Older adult lives alone 7. Concerns about memory 9

10 10 Example: Managing Injurious Falls Provide information and educate Determine reason for fall including environment and medications Make an action plan to prevent future falls Provide assisted referral to Matter of Balance class. Arrange ride. Follow and track results
