3 BACKGROUND Burns Lake Hospital is a 13-bed acute care rural hospital –With a full-time “stand-alone” Pharmacy Technician One of Northern Health Authorities’ Medication Reconciliation test sites

4 AIMS To prevent Adverse Drug Events (ADE’s) by implementing medication reconciliation at admission in Burns Lake Hospital Goals: –To increase the effectiveness of the medication admission reconciliation process over time –To reduce the rate of Unintentional and Undocumented Intentional discrepancies at admission by 75% in 6 months
