4 Transition Process e By February 1 of each year PSEI programs must identify children who are approaching age for K or 1 st grade in the their SD of residence. Use Notices e The PSEI records of child who are no longer eligible for special education services cannot be shared with SDs or charter schools without parental consent. 6 Transition Meeting ePSEI program must convene transition meeting by the end of February for all children approaching age for K or 1 st grade. eMust provide appropriate Intent to Register form ePSEI program reviews choices on this form with families eSD or charter school responsibilities commence with receipt of Intent to Register form

8 Reevaluations and the IEP eIf parents intend to register with the SD or charter school the Notice of Options for Your Child’s Transition is reviewed with the parents by the school district team or the charter school team. eWithin a reasonable period of time from receipt of the Intent to Register form but no later than April 15, the SD or charter school will notify the parent in writing and initiate one of the options.

9 Remain in Early Intervention EI IEP Team must: eFully inform parents of advantages of transitioning to school age program with same age peers eEvaluation/reevaluation must reflect an additional year of Early Intervention at the request of the family

Waiving the Reevaluation eOccurs prior to the reevaluation and NOT part of a reevaluation eParent and school district or charter school may agree that a reevaluation is unnecessary – NOT an IEP team decision eParents must agree in writing using Agreement to Waive Reevaluation form

17 Early Intervention: Developmental Delay eA child in Preschool Early Intervention with a primary disability of developmental delay should be reevaluated to determine if the child has a school age eligible disability and if she/he does not they should be graduated from Early Intervention. ePrior to a child receiving school age special education the child must be determined eligible by one of the disability categories. eJacob M. requires that special education services cannot be interrupted when children transition from Preschool EI to school.

18 e For all disability categories except for Developmental Delay, a child who transitions into K without an evaluation, the first evaluation in the school-age program would be considered a reevaluation unless the child was exited from the EI program via a NOREP/PWN. e When a child with a primary disability of Development Delay transitions into a school age program with no evaluation and an evaluation occurs in the school-age program it is considered an initial evaluation. e For all disability categories except for Developmental Delay, a child who transitions into K without an evaluation, the first evaluation in the school-age program would be considered a reevaluation unless the child was exited from the EI program via a NOREP/PWN. e When a child with a primary disability of Development Delay transitions into a school age program with no evaluation and an evaluation occurs in the school-age program it is considered an initial evaluation.
