2  Offers a support system to help patients live as actively as possible until death  Offers a support system to help the family cope during the patient’s illness and in their own bereavement  Uses a team approach to address the needs of patients and their families, including bereavement counseling, if indicated Palliative Care

3  Will enhance quality of life  May positively influence the course of illness  Is applicable early in the course of illness, in conjunction with other therapies that are intended to therapy, or antiretroviral therapy and includes those prolong life, such as chemotherapy or radiation investigations needed to better understand and manage distressing clinical complications

5 PALLIATIVE CARE HAS TWO ESSENTIALS  Supportive care (present in many home care services)  Pain and symptom control using the methods researched since 1967  The following are not palliative care:  Supportive care without pain and symptom control using the researched methods  Pain and symptom control without supportive and holistic care

6 CCW Provide Access to Care  Palliative components gives special care for the impoverished and under served with emphasis on those falling through the cracks  Patient identification, support, referrals, education….

7 Inter-disciplinary Team  CCW as PART of a team (Dr, Nurses, Social workers, OT, physio, spiritual, etc)  More than multi - patient moves between all as required  A professional nurse supervises HBC delivered by trained CCW, caregivers and volunteers

8 Pain and Symptom Control  Is the core of CCW work  CCW taught to use a specific tool to categorize for referral and support  High quality training essential  Cultural norms are respected

9 Holistic Care  Patients AND their families have access to physical, psycho-social and spiritual care

10 Pediatric Palliative care  Appropriate plans are developed and initiated with regard to the care of children

11 Bereavement Care  The bereaved are supported during the process of mourning and their adjustment to loss  (includes wills, memory boxes, etc)

12 Care of Caregivers  A program for providing holistic support to community caregivers and volunteers is incorporated into HBC

13 The need for palliative care in our country has never been greater… The need for acknowledged and well trained and supported CCW in our country has never been greater…
