9 Three Indices I. Adverse Outcome Index (AOI) - # patients with one or more adverse outcomes / total number of deliveries II. Weighted Adverse Outcome Score (WAOS)- total weighted adverse outcome score/ total number of deliveries III. Severity Index (SI) – total weighted adverse outcome score/ # of patients with an adverse outcome

10 Data Submission Process Administrative and Technical Contact Business Associate Agreement and Contract Baseline Data 2-4 quarters of CY 2006 data Monitoring Data: 2-4 quarters of data starting with 10/1/07 discharges Administrative discharge abstract data with supplemental perinatal variables

11 Data Files- Core Data Set Hospital IS Department/HSCRC or DCHA data file MDC 14 or all discharges 0-28 days old admission Electronic submission to secure website
