5 Introduction The purpose of the RFA is to solicit applications from eligible organizations to address the social, health, educational, and economic consequences of adolescent pregnancy. The successful applicant will effectively serve the greatest number of pregnant and parenting teens in their local area that are most likely to benefit from a client- centered, strength-based, positive youth development program that supports the goals of the AFLP. 6 General Program Requirements The goals of the AFLP are to: Improve the health of the pregnant and parenting teen, thus supporting the health of the baby Improve graduation rates for pregnant and parenting teens Reduce repeat pregnancies for pregnant and parenting teens Improve linkages and create networks for pregnant and parenting teens Maternal, Child and Adolescent Health Division November 16, 2011

7 Target Populations The AFLP targets a high risk population that also has the greatest potential to benefit from the AFLP. The AFLP Policy and Procedure Manual (Appendix 2) provides a list of possible risk factors to be used to assess and prioritize client enrollment and eligibility for continued services. 8 Client Eligibility Eligible clients include: The adolescent female who is pregnant or parenting up to age 19 years of age and are most likely to benefit from the program. A male who is an expectant parent or parenting father up to age 19 and most likely to benefit from the program is also eligible. Clients may receive services up to their 19th birthday, or for 24 months, whichever is longer. Maternal, Child and Adolescent Health Division November 16, 2011

9 Funding Availability AFLP funding is based on a level of $1697 per client (teen and their child) for twelve months using a case management model of services that support the AFLP goals and objectives. Estimated total funding to be allocated among grantees through this RFA is $21,799,410 over three years, contingent on future federal Maternal and Child Health (MCH) Title V Block Grant funding. 10 Funding Availability (continued) Funding is limited to the following amounts allocated annually for the duration of this program: 1.$7,266,470 for the budget period of 7/1/12-6/30/13 2.$7,266,470 for the budget period of 7/1/13-6/30/14 3.$7,266,470 for the budget period of 7/1/14-6/30/15 Maternal, Child and Adolescent Health Division November 16, 2011

11 Matching Fund Requirement Funding available through this RFA is not intended to provide reimbursement for an applicant’s total cost. There is no specific percentage required for the applicant’s contribution. In-kind matching is allowed. Maternal, Child and Adoescent Health Division November 16, 2011

12 Number of Awards The CDPH expects to award contracts to eligible and qualified Applicants that will: Serve the target population in California counties with the highest need; see Health Status Indicators (Appendix 4) Demonstrates the greatest capacity to achieve program goals Contracts will be for a three year period. CDPH reserves the right to determine the level of funding to be awarded within the available funding and considering the need of the community. 13 AFLP Program Requirements Applicants must comply with the following: AFLP Scope of Work (Appendix 1) - describes the program goals, objectives, activities and evaluation measures for the contract period. AFLP Policy and Procedure Manual (Appendix 2) - provides direction in the implementation of AFLP, in conjunction with the Scope of Work MCAH Fiscal Policies and Procedures Manual (Appendix 3) - describes the process for allocation of funds used to assist agencies in promoting the health of California’s mothers and children, consistent with the federal MCH Title V Block Grant. 14 Program Activities AFLP uses a comprehensive case management model to assess and address the risks and resources of adolescent teens and their children. Agencies assign a case manager who works with the teen clients to identify the teen’s goals for the future, assess the teen’s needs as well current strengths and resources. 15 Program Activities Case managers work with the teen to develop a plan of action for achieving health, educational success, economic self-sufficiency, healthy relationships, and becoming a productive member of her/his community. Agencies also develop linkages and create networks among community resources for support of the pregnant and parenting teen Maternal, Child and Adoescent Health Division November 16, 2011

16 General Applicant Information Eligible Applicants: Applicants must have a minimum of three years experience providing case management or other social services to pregnant and parenting teens and be either a: City, county, other governmental body or special district State/public college or university Non-profit health, social service or community based organization, hospital, or community clinic Maternal, Child and Adolescent Health Division November 16, 2011

17 Ineligible Applicants Commercial businesses operating on a for-profit basis Organizations deemed ineligible by the Department of Fair Employment and Housing because of failure to comply with CA’s nondiscrimination laws and reporting requirements Organizations debarred or decertified from contracting by the federal government Organizations that promote sectarian beliefs related to the doctrine of a religious group Organizations unable to comply with the Sexual Health Accountability Act of 2007 (H.S. 18 Voluntary Letter of Intent The Applicant is asked to voluntarily identify their intent to submit an application (see attachment 5) This request is made solely to assist the MCAH Division in planning staffing needs related to the RFA Review. Failure to submit a Letter of Intent will not affect the acceptance of any application nor is a Letter of Intent binding in any way. Maternal, Child and Adoescent Health Division November 16, 2011

Each section has been assigned page limits and scoring values. Applicants are to develop applications by following all the RFA instructions. 23 Application Review Process The application review is a two phase process Phase 1 - Applications will be reviewed to determine if the Applicant meets the initial eligibility criteria: – Type of entity or Organization – Type and Length of Experience Applicants who pass the initial review phase will move to the second phase of the review.
