TLAP and the Shared Commitment: Integration and personalisation TLAP Conference, 25 th September 2013
The connections... Integration is a means towards an end, not an end in itself: Personalisation: A means towards ensuring that the health and care system is built around the needs and aspirations of people with care and support needs, which values their contribution and empowers them to exercise the choice and control that is right for them. Prevention: A means towards redesigning the system so that ill health and functional impairment can be avoided, or managed much more effectively, and people can live in stronger, healthier communities. Broader focus on health and wellbeing: A means towards tackling those issues outside the reach of health and social care, which undermine peoples health and wellbeing or can contribute to improved health and wellbeing in the future. Coproduction: A means towards engaging people in the design, delivery and evaluation of services, in assessing their needs and planning which treatments and supports would best meet them.
