4 International Atomic Energy Agency 4 Key points from presentations, posters and panel discussions Chemical Industry experience Risk of hydrogen detonation - high even at concentrations considered to be “safe” Prevention of hydrogen accumulation – venting and catalytic oxidation Post-operational phase (Germany experience) Risks created by specific conditions in post-operational phase Examples - modifications of systems, modified operation modes, increased decontamination, reduction of personnel including shift staff, staff training etc.

5 International Atomic Energy Agency 5 There is a need to improve sharing of important OE by MS through IRS. The actions taken after events should be considered in back-fitting programmes in other MS. Feedback from MS on how important lessons learned were used to prevent similar issues needs to be improved. MS are encouraged to develop effective self- assessment programmes supported by strong leadership, creating positive attitude to OE and engaged employees in continuous improvement. MS are encouraged to benchmark with non-nuclear organizations (e.g. aviation, chemical industry etc) with the objective to share OE and improve operational safety Potential areas for further improvements

6 International Atomic Energy Agency 6 The design of future reactors should also make optimal use of accumulated OE. Good practices should be identified and shared to serve as examples. The effectiveness of OPEX on regulatory oversight needs to be more strengthened. The international OE feedback systems should be supported by an internationally agreed mechanism for coordination and guidance. There is a need to continuously improve the effectiveness of OE by enhancing RCA and monitoring of corrective actions. Potential areas for further improvements
