12 Reporting Test Scores Reporting Test Scores to Colleges Eastview High School does not release student test scores to colleges, NCAA, or scholarship agencies. It is each student’s responsibility to release official ACT or SAT test scores if they are not requested at the time of test administration. Consider using the on-line process to release scores by going to www.act.org or www.collegeboard.com.www.act.orgwww.collegeboard.com Deadlines: During peak periods, college applications can take more than ten school days to process. Students are responsible for knowing deadlines and bringing application materials to the Guidance office in time for proper processing. Dates to Remember: November 1 - Deadline for most Early Decision or Early Action applications December 15 – Deadline for the University of Minnesota (Completed application submitted to EVHS Guidance Office on or before 11/30/05.) February 1 – Deadline for the University of Wisconsin (Completed application submitted to EVHS Guidance Office on or before 1/13/06.)

23 The College Application Process Many colleges prefer applications be sent through an on- line process. However, most will still accept the paper version if it is available. Application fee payments can be made on-line or by check if sent by regular mail. Students are responsible for submitting their own portion of an application either on-line or through the mail. If a college requires a school or counselor report form as part of the application, this form must be submitted to the Guidance office for processing. Counselors will complete the form and mail it with a copy of the student’s senior year schedule and recommendation letters if required.

27 Complete Application Routing Form College Application Routing Forms, available in the Guidance office, must accompany each School or Counselor Report Form submitted. Students must indicate the following: 1) college or university name, 2) application deadlines, 3) which transcript was released - honors or regular, 4) which test score was released – ACT, SAT, or both, including the test date. Mid-year reports forms, if required, should be given to Pat Gerrits in the Guidance office. They will be completed and mailed after first semester grades become available. (It is not necessary to release another official transcript.)
