3 Problem Older adults migrate to Arizona to get away from colder climates. In doing so they may leave behind: Medical records Familiar doctors Vital documents Comfort of local healthcare SNAP Health was built to address the issue of lack of web-based health resources for travelling older adults, our team has developed a web application that seeks to: Create a single place to make quick medical decisions and get access to health information in a SNAP. 3www.mysnaphealth.com

4 Background Research There is currently no site for winter travelers to access local health information. Current techniques for gathering information include – Calling home health agencies – Using the phone book – Getting friend referrals An article from the American Medical Association describes the way doctors have to get emergency information through Fax! 5 Potential for Impact SNAP Health will use data from HHS and other sources to help these visitors make informed decisions about seeking care in their vacation state and store some basic health information for quick access in case of emergency. HHS Data Used – 30 day mortality rates for Pneunomia, Heart Failure, and Heart Attacks by state. 8 Next Generation and Scalability www.mysnaphealth.com8 Designed so that when the patient is heading to another area, they can select which healthcare providers have permission to access the patient’s information. The healthcare facilities will have information on what insurance is accepted to help aid the patient in their decision of where to receive care. Links to: -Travel/booking sites -Car rentals -Local coupon deals. Store legal documents for quick access by medical professionals in an emergency Background login for practitioner so that they can see information that the patient has marked as public. Temporarily store medical records for travelers in case of emergency.
