2. Hamilton Enrolls assists patients to enroll in: Medicaid, CHIP Medicare Part D, Low Income Subsidies, Pace/Pacenet Presumptive eligibility for pregnant women Our enrollment and inreach efforts have assisted over 5,000 patients per year for the past 10 years.

3. Hamilton has received Certification as an Application Counselor Organization Five staff have completed the curriculum, passed the required tests and are now Certified Application Counselors (CAC). Certificates are displayed The CAC Standard Operating Procedures Manual is available for reference and support A separate telephone line was set up that allows direct contact with our counselors A referral/Resource sheet has been developed to assist consumers with their needs Preparing for the the Affordable Care Act

4. Education and Enrollment Activities Uninsured patients who are seen by a provider are put on the CACs’ schedule to be informed and assisted with enrollment into eligible insurance options Appointments are scheduled to assist with enrollment Group educational sessions are being scheduled for our patients as well as the general public We have partnered with The United Way of the Capital Region and Pinnacle Health System to provide services to consumers as needed We will also work with churches, temporary help agencies, unemployment agencies
