2 The Healthy Start Model Purpose: To reduce disparities in infant mortality and adverse perinatal outcomes. Approaches: – Improve Women’s Health – Promote Quality Services – Strengthen Family Resilience – Achieve Collective Impact – Increase Accountability through Quality Improvement, Performance Monitoring, and Evaluation Supporting HRSA Goals: 1.Improve Access to Quality Health Care and Services 2.Strengthen the Health Workforce 3.Build Healthy Communities 4.Improve Health Equity

3 Level 1 Community-based Healthy Start Community Collaboration, information sharing and advocacy Responsible for Individual Effects Program Participants 500 Providers & Partners Community Action Network

4 Level 2 Enhanced Services Healthy Start Responsible for Community Level Effects Community Collaboration Community Partners Program Participants 800 Common Agenda, shared measurement And coordinated resources
